When your loved one joins our Lexington, South Carolina, Memory Care community, we will have a plan in place designed with the personal care services that best meet their current needs and abilities. Every 90 days (or sooner if needed or desired), our highly skilled care team will reassess the plan to make certain your loved one is receiving the individualized care that they deserve. Our team of nurses and leaders want to ensure that your loved one’s cognitive plan evolves with changing health conditions.
